On May 15th, students will be able to get Athletic Physicals at FCMS during the school day. The physical will cost $25 with payment via cash, check (made out to ReGenesis Health Care), or Card. Any student can take advantage of this opportunity. These physicals will be valid until May 31, 2025.
Students will not/can not be seen without the following forms being submitted BEFORE the physical date.
