• In preparation for summer workouts and the 2023-2024 school year, information about athletic opportunities for students attending D.R. Hill Middle School are below.  If you have questions, feel free to email zachary.johnson@spart5.net.

     

    ATHLETIC PHYSICAL REQUIRED 

    Physicals must be dated April 1, 2023 or later to be valid for the 2023-2024 school year. 

    To be eligible for athletic tryouts, follow these steps:

    • Pick up a copy of the athletic physical form from the D.R. Hill office.

    • Go to an urgent care, minute clinic, or your personal physician and have the physical form completed - must be signed and dated by a physician 4/1/2023 or later.

    • Log on to planeths.com and create an account

    • Upload athletic physical forms (required for workouts/tryouts)

    • Other documents on planeths are required upon making a team.

    Paper copies provided by athletes will not be accepted this year.  Parents should upload a valid physical into planeths prior to athletes showing up for a workout or tryout.
